NEW BUSINESS QUOTING, PLAN CHANGES & COMMISSIONS

For agents requesting a quote for the SBAM sponsored Dearborn National life and disability program on behalf of your customer, please click through below for our quoting tool or contact the Managing Agent in your area.

To submit your new business enrollment, please send the completed Employer Transmittal and Employee Enrollment forms along with a check in the amount of the first month's premium made payable to 'The SBAM Plan' to the Managing Agent in your area.
