Communicating with your COBRA coordinator

We make it easy to communicate with your COBRA Coordinator. We're available Monday through Friday from 8 am until 5 pm and we will return your call or email within 2 hours on any given business day. 

To notify your COBRA coordinator of any changes within your plan, just fax or email the Enrollment/Change of Status (ECOS) form to us along with our customized COBRA Cover Sheet and your COBRA Coordinator will determine if the event qualifies for COBRA. If so, they will send the necessary notifications within the allotted time in order to stay compliant with the law. Examples of some qualifying events include: new hire, termination, death, marriage, and divorce.  

We offer two convenient ways for you to send your ECOS and COBRA Cover Sheet:

If your company is enrolled in an SBAM-sponsored Blue Cross Blue Shield of Michigan or Blue Care Network plan, our billing department will automatically receive a copy of your ECOS form to make any necessary billing and plan changes - whether you fax or email the information.

Private Health Information

Please help us help you stay in compliance with federal laws revolving around Private Health Information and in keeping your plan records accurate.

Private Health Information:
Two federal laws, The Health Insurance Portability and Accountability Act (HIPAA) and the Health Information Technology for Economic and Clinical Health (HITECH) Act require that confidential personal health information that is transmitted through or stored on the internet must be protected as it flows within and across organizations.

When you send us information about your employees or COBRA qualifying individuals — including name, address, Social Security number, driver’s license number, or health plan information — in order to stay compliant with federal law you must send it through our secure fax or email. You should not send your employees' private health information through a general email account or fax line that offers no protection.

COBRA and Health Plan Records Administration:
An added reason to use our secure connections. When using our secure connections, you are also communicating with our billing administrator, Grotenhuis, so that they may keep your SBAM sponsored health plan enrollment and billing records current. This is a separate process that is in addition to the COBRA administration process here at SBAM. Using the secure connection lets Grotenhuis know who to add to, or terminate from, your coverage.
