Self-Administered option: $99 to create / change documents
Fully Administered option: $27.50 per month

Flexible Spending Accounts
$5.25 per month per participant; free debit card 
(minimum of $52.50 per month)

Health Savings Accounts
$2.75 per month per participant; free debit card
(minimum of $27.50 per month)

Health Reimbursement Arrangements
Standard: $5.25 per month per participant; free debit card
