Employers may receive either a group Policy or a Certificate of Insurance containing a detailed description of the insurance coverage under the group Policy. Life and AD&D

Employee-paid voluntary term life coverage available in $10,000 increments from $10,000 to $500,000, not to exceed 5x annual salary; $100,000 employee guarantee issue amount; spouse and child coverage options included; AD&D matches term life benefit; portability and conversion are included.

Group Long-term Disability Plan Highlight– 60% of basic monthly earnings, up to $6,000 per month; 90-day elimination period; maximum benefit period is 5 years to Social Security Full Retirement Age

Group Long-term Disability Plan Highlight – 60% of basic monthly earnings, up to $6,000 per month; 90-day elimination period; maximum benefit period is Social Security Full Retirement Age

Group Short-Term Disability

Group Short-term Disability Plan Highlight – 66 2/3% of basic weekly earnings, up to $1,000 per week; elimination period is 0 days for injury and 7 days for illness, subject to sick leave/salary continuation; maximum benefit period is 13 weeks.

Group Short-term Disability Plan Highlight – 66 2/3% of basic weekly earnings, up to $1,000 per week; elimination period is 0 days for injury and 7 days for illness, subject to sick leave/salary continuation; maximum benefit period is 26 weeks.

Group Short-term Disability Plan Highlight – 66 2/3% of basic weekly earnings, up to $1,000 per week; elimination period is 14 days for both injury and illness, subject to sick leave/salary continuation; maximum benefit period is 11 weeks.
