The SBAM Wellness Program is designed to be simple and fun, and keep your employees engaged. Our employee wellness solutions benefit not only your staff, but your entire organization. Our goal is to reach participants where they are when they need it. The combination of our online wellness portal, programming options and incentive strategies will increase the value of your biggest investment – your people!

Each employee will register their own personal account on the wellness platform and regularly visit the site to find important wellness information, access challenges, track activity and use specific tools that are geared towards their health risks. Purchase the Wellness Program now.

SBAM Basic, Premium, VIP and Elite level members can participate! The cost for the SBAM Wellness Program is a $250 implementation fee and $7.50 per participating employee per month.

Click below to go through the steps to pay the initial $250 implementation fee for your company. You must be logged in as a paid SBAM member to view the purchase options. Then submit the employee census to our OnSite Wellness, LLC representative, Allison Cobb.

Reward & Incentive Options

Your company has two options for incentives to offer your employees; Rewards Mall or Incentive Points. See below for details.

Wellness Program

Participants are eligible for an incentive if they earn on average 100 points per month. The goal is for each person to earn 300 points each quarter to claim their prize.

Incentive can include:

  • Raffle prizes which are selected by you as a company.
  • A company selected prize for all participants that reached the quarterly point value.

*All prizes are the responsibility of the company to purchase and distribute.

Wellness Program

Wellness Program Plus Rewards Mall

  • Access to electronic gift cards that include Amazon, Apple, Best Buy, Hotels.com, Starbucks, Target, The Home Depot, Visa, Walmart, Whole Foods Market and donation options to Clean Water Fund and Habitat for Humanity.
  • Point ratio where 10 wellness points = $1 (see point sheet for reference).
  • Minimum of 250 points ($25) must be earned to open the rewards mall.
  • Maximum of 1000 points ($100) is eligible for redemption per participant per program year (annually).
  • Custom options that may carry additional fees include tangible prize options such as logo wear, PTO, HSA contribution, and more. Please note that the custom options are reported to the company up to once per month and are the responsibility of the company to purchase and distribute.
  • Gift card options available can vary per company preference, all options do not need to be available.
