100% participation of all eligible employees for the basic life, short and long term disability coverages is required.  Basic life, short-term disability and long-term disability can be written on a stand-alone basis.

Customer Service & Consolidated Billing

Our consolidated billing and enrollment service brings you exclusive access to our responsive customer service team.  Our representatives are just an email or phone call away to assist with billing, enrollments, terminations, new ID card orders and more.

Additionally, premium payments for group health, dental and vision, group life and group disability, administration fees for COBRA and Section 125, FSA, HSA and HRA services will come on a single consolidated invoice from SBAM once per month.

This consolidated billing service allows you to make payment for SBAM services jointly (one check) and just as important, when you add or terminate an employee, you only need to communicate that once through our HIPAA secure Zix email. Not only can we guarantee a simple consolidated invoice, we’re the only organization that has the capability to put Blue Cross and BCN premiums on the same invoice.

*There is a $7.50 monthly billing administration fee that is included on your statement.
